Lens: EF28-70f 2.8L - at 70mm

Exposure: f4.5, 1/200, ISO200

Lighting

Studio flash. Single flash head to the right to give strong side light.

Post Processing (Photoshop)

This is actually an Adobe Lightroom preset adjustment; the image is partially desaturated, the contrast level increased and the tone curve boosted to increase the contrast more. I also brightened the eyes a little.
