This photograph of Natalia Forrest was taken using studio lighting.

Camera: Canon EOS 40D
Lens: 28-70mm f2.8L
Exposure: f4, 1/200, ISO100
Lighting
Two softboxes in front at 45 degrees to the model and a snooted hair light behind.
Post Processing (Photoshop)
There wasn't much photoshop work required; it was converted to black and white and the contrast boosted with a curves adjustment. I also added a touch of glamour using my airbrushing action.
